While IUI is a popular and less invasive first step, it is important to know that it brings your chances of pregnancy closer to that of a fertile couple trying naturally, but it does not guarantee a baby on the first try.

General Success Rate of IUI

On average, the success rate for IUI ranges from 10% to 20% per cycle.

This number might seem low at first glance, but it is important to look at the bigger picture. A healthy couple with no fertility issues has about a 20-25% chance of getting pregnant naturally each month. IUI aims to restore these odds for couples facing difficulties.

However, the real power of IUI lies in persistence. Since the procedure is affordable and simple, most couples try more than once. The cumulative success rate (the chance of pregnancy after trying multiple times) is much higher. After 3 to 4 cycles of IUI, pregnancy rates can reach as high as 40% to 50% for suitable candidates.

Key Factors That Influence Your Success

  1. Age of the Female Partner

This is the single most significant factor. A woman’s egg quality and quantity naturally decline with age, which directly impacts IUI outcomes.

  • Under 35 years: Highest success rates (approx. 15-20% per cycle).
  • Ages 35 to 40: Success rates begin to dip (approx. 10% per cycle).
  • Over 40 years: Success rates drop significantly (often below 5% per cycle). For women in this group, moving directly to IVF is often recommended to save time.
  1. Sperm Quality (Post-Wash Count)

While IUI helps weak sperm, it still requires a minimum number of healthy swimmers.

  • We look at the “Total Motile Count” after the sperm has been washed in the lab.
  • If the post-wash count is above 10 million, success rates are excellent.
  • If the count is between 5 to 10 million, IUI is still effective but success rates may be slightly lower.
  • If the count is below 5 million, the chances of IUI working are very low, and other treatments may be better.
  1. The Cause of Infertility
  • Anovulation (PCOS): Women who do not ovulate regularly (like those with PCOD/PCOS) often see excellent results with IUI because the medication helps release the egg, and the procedure ensures sperm is present at the right time.
  • Unexplained Infertility: IUI works well for couples where no specific cause is found.
  • Endometriosis: Women with mild endometriosis have good success rates, but severe cases (where tubes or ovaries are affected by scarring) see lower success with IUI.
  1. Follicle Count and Lining

The cycle is more likely to succeed if:

  • You produce 2 healthy follicles (eggs) instead of just one (though this increases the chance of twins).
  • Your uterine lining (endometrium) is thick and healthy (usually above 7-8mm) to support the embryo.

Medicated vs. Unmedicated Cycles

Does taking medicine improve your chances?
Yes.

  • Natural Cycle IUI: This involves no medication and follows your natural ovulation. Success rates are generally lower (around 6-10%).
  • Medicated IUI: Using oral tablets (like Clomiphene or Letrozole) or low-dose hormone injections to boost egg production significantly raises success rates to the 15-20% range. This is the standard protocol for most patients in India.

The “Cumulative Success” Effect

Think of IUI like rolling a dice. You might not get the number you want on the first roll, but if you roll three or four times, your chances of getting that number increase.

  • Cycle 1: ~15% chance
  • Cycle 2: ~25-30% cumulative chance
  • Cycle 3: ~35-40% cumulative chance

Most pregnancies resulting from IUI happen within the first 3 to 4 cycles.

When Does the Success Rate “Plateau”?

It is crucial to know when to stop. Research shows that if IUI hasn’t worked after 3 or 4 attempts, the likelihood of it working on the 5th or 6th try drops drastically.

At this point, continuing with IUI may not be the best use of your time or finances. We usually recommend a consultation to discuss moving to IVF (In Vitro Fertilization), which has much higher success rates for couples who have not had success with IUI.
